Quick Apply Full-time 9 hours ago Full Job Description Emory Valley Center seeks a DIDD Residential & Community Program Manager to join our team.
This position is full time and the salary is 45K annually.
The incumbent will assist, train, and supervise people that support individuals with intellectual & developmental disabilities in activities of daily living.
The incumbent will also serve as an advocate & liaison for the people supported at the agency, ensuring excellent coordination of care and services.
$250 stipend for assigned on-call weekend $100 for any shift work picked up beyond the basic work week $2000 sign on bonus Job duties:
Maintains communication between service provider, families, conservators, Independent Support Coordinators (ISC's), State Case Managers, medical professionals, therapists, advocates and MCO's regarding services, supports, progress, and problems.
Provides short-term crisis intervention for families and people supported, and advocates for program changes and/or additional supports, as needed.
Writes and updates Individualized Support Plans for people on caseload.
Develops specialized support plans as needed to address specific behavioral, mental health, medical and other concerns.
Has a clear and concise understanding of DIDD, HUD, MCO and other regulatory agency licensing requirements and ensures that all sites operate within the agency exceed basic licensure guidelines.
Monitors documentation of medication, daily notes, individual training specifics to the person (ITSP's), Individual Support Plan's (ISP's), Person-Centered Support Plans (PCSP), and monthly billing to ensure accuracy.
Completes home visits and day service visits for assigned caseload.
Works a flexible schedule being available in the afternoons for the operational sites.
On-call responsibilities to handle medical emergencies or those requiring professional medical or staffing support- this will include holidays and weekends in cooperation with other management staff.
Follows all therapy plans (OT, PT, SLP and Nutrition) and Behavior Support plans as defined in ITSP, ISP/PCSP or competency-based trainings.
Acts as a designated trainer for competency-based plans- trains staff and ensures competency.
Supervises, evaluates, and trains Home Managers and Direct Support Professionals.
Completes and assists with staff annual performance evaluations and offers support and coaching to staff.
Responsible for interviewing, and recommending applicants for hire to Human Resources for available positions in program.
EDUCATIONAL AND WORK EXPERIENCE REQUIREMENTS:
High school diploma or GED equivalent with 4 years experience in DIDD field or related field.
Associate or Bachelor's Degree in related field preferred.
Minimum of 1 year of supervisory experience preferred.
Possess Valid TN Driver License & acceptable driving record.
Excellent scheduling and time management skills.
Advanced computer skills.
Ability to lift 50 pounds (weight of average manual wheelchair when chair is empty).
